The following information must be provided on the application:

Business Information: Business name and address

Owner/Partner Information: Name, title, phone number, email and address of all owners/partners

Primary Contact Information: Name, title, phone number, email and address (if different from the business address)

Insurance Information: Insurance provider name, policy number, policy start and end dates, phone number, address and proof of insurance (upload required)

The annual solid waste hauler license fee is $1,000 per vehicle and is valid through the end of the fiscal year, which runs from July 1 to June 30. Applications and payment for renewals or new licenses are due 30 days prior to the start of the new license period. Fees are prorated based on the month the license becomes active.

A license is issued upon the City of Tempe’s approval of the application and payment and is valid through the end of the fiscal year, starting:

• from the beginning of the fiscal year for renewals and full year licenses, or
• from the submission month for mid-year licenses.

To qualify for a Tempe Solid Waste Hauler License, the following insurance coverage is required:

• Personal Injury Coverage: Maintain $1,000,000 per individual.
• Incident Coverage: Maintain $1,000,000 for all persons in a single incident.
• Property Damage: Maintain $500,000 per incident.
