Welcome to City of Tempe Special Olympics!
Youth (8 yrs+) and adults with intellectual and developmental disabilities are invited to join Special Olympics Arizona to learn sportsmanship and teamwork while achieving their personal goals.
Participating in City of Tempe Special Olympics requires the following:
1. Registering for the sport you want to participate in
2. Pay the $10.00 fee per sport
3. Completing a Special Olympics Arizona Medical Waiver and Consent Form (with doctor signature)
Starting January 2020 we will no longer be accepting registration and payments at practices. In order to participate you must register and pay before the sport online at www.tempegov.brochure or you can come into any City of Tempe Recreation office and submit your registration and payment.
HERE is a list of locations you can drop your registration and payment off to but we recommend dropping it off at the Adaptive Recreation Office, 3500 S. Rural Rd. Suite 201, Tempe, AZ 85282
Completed and Signed Medical & Consent Forms
All Special Olympics athletes are required to have an up-to-date Medical Release and Consent Form on file with the City of Tempe delegation and Special Olympics Arizona in order to participate in official Special Olympics Arizona competitions and events. The medical and consent forms contain sections that must be filled out and signed by both the athlete (or parent/guardian) AND a doctor. This document is good for three years from the date of the doctor's and participant's (or guardian's) signatures. Athletes without an up-to-date Medical Release and Consent Form will not be allowed to participate in Special Olympics competitions, however, an athlete is welcome to continue participating in the practices without being registered for the competitions. Please notify Adapted Recreation at the start of the program if you choose to do this.
Filled out registrations and medical & consent forms can be mailed to City of Tempe Adaptive Recreation, 3500 S. Rural Rd. Suite 201, Tempe, AZ 85282, or they can be submitted in person at the Recreation Services Office at the same address on the second floor of the Tempe Public Library.
City of Tempe currently offers nine Special Olympics Arizona sports throughout the year, with each sport taking place during a specific season. The sports offered are:
Winter Season - November 2019 - February 2020
Spring Season - February 2020 - May 2020
