Pursuant to 40 C.F.R. 403.12, Tempe City Code
§27-54, and Part 3 of your wastewater discharge
permit, all permitted Industrial Users (IU) are
required to complete an annual report and, in
addition, those industries classified as
Significant Industrial Users (SIU) are
required to complete semiannual reports pursuant
to 40 C.F.R. 403.12(e).
The report
forms are only available in portable document
format (PDF).
However, all
PDF documents issued by the Environmental
Services Section can be saved if you are using
version 8 of AdobeÓ
Reader, which can be down loaded from
http://www.adobe.com.
The 2011 Annual Report is
due by the close of business on February 24,
2012. As stated previously, the Annual
report applies to all Industrial Users ("IU").
