Pursuant to 40 CFR 403.12, Tempe City Code
§27-54, and Part 3 of your wastewater discharge
permit, all permitted Industrial Users (IU) are
required to complete an annual report and, in
addition, those industries classified as
Significant Industrial Users (SIU) are
required to complete semiannual reports pursuant
to 40 CFR 403.12(e).
The report
forms are only available in portable document
format (PDF).
However, all
PDF documents issued by the Environmental
Services Division can be
saved if you are using version 8 of AdobeÓ
Reader, which can be down loaded from
http://www.adobe.com.
The 2009 semiannual report is
now available below and is due by the close of
business on August 5, 2009. This
report applies to the SIUs only. The annual report
forms will be posted in early December of 2009,
and will be due on January 29, 2010. An email will be issued to each
SIU/IU,
providing a link to download the appropriate
report form
from a secure City of Tempe site.
