Pursuant to 40 CFR 403.12, Tempe City Code
§27-54, and Part 3 of your wastewater discharge
permit, all permitted Industrial Users (IU) are
required to complete an annual report and, in
addition, those industries classified as
Significant Industrial Users (SIU) are
required to complete semiannual reports pursuant
to 40 CFR 403.12(e).
The report
forms are only available in portable document
format (PDF).
However, all
PDF documents issued by the Environmental
Services Section can be
saved if you are using version 8 of AdobeÓ
Reader, which can be down loaded from
http://www.adobe.com.
The 2010 Annual Report is due by the close of
business on August 06, 2010. As
stated previously, this
report applies all permitted Industrial
Users, both IU and SIUs.
All necessary forms are available below.
