Missing Cell Phone Form

If you lost your cell phone, please use the following form.  If your phone was stolen and you know who took it, please contact us at 480-350-8311.  Please make sure the lock is showing on the bottom to indicate this is a secure page.

If you have contacted your cell phone provider and calls have been made from your phone since it was last seen, please contact us at 480-350-8311.  An officer will need to make a report for you.


The phone must have been seen for the last time at a Tempe location.  If it was last seen in another city, you must contact that police agency.  You will need the exact location, date and time you believe you lost it.   If you have questions, please contact the Tempe Police non-emergency number 480-350-8311. 


An incident number will be emailed to you within 2 days.  You will not be contacted in person unless additional details are needed.  If you do not receive an incident number within two days, please contact 480-350-8311.
