Wave Pool Admissions

You may add Wave Pool admissions to your party if your party is held during our public Wave Pool sessions. Please check www.tempe.gov/pools/KiwanisWavePool for hours. Note: Children under the age of 8 must be accompanied in the water by an adult at all times.

IMPORTANT!

Note that filling out this form does not guarantee that you have reserved the party, however it is a contract.  Please make sure your contact information is correct before submitting this form. 

After submitting this form, we will contact you as soon as possible to confirm your reservation, or to let you know that we are unable to fulfill your request at this time. 

If we are able to fulfill your request, a $60 non-refundable/non-transferable deposit will be required at the time of our confirmation call in order to hold your reservation.
The following rules and regulations apply to your birthday party, please read each and check the box to indicate you have read and agreed to the following terms:
Parties are limited to 12 children, additional children are $10 each.
There is no charge for adults.
You may bring a birthday cake (no ice cream or ice cream cakes). If you do choose to bring a cake you will need to bring all items needed to serve the cake (i.e. paper plates, forks, candles, matches, knife to cut the cake).
Other food and beverages are not permitted, unless you have selected the option above.
Food packages should be confirmed 7 days prior to your party.  All food ordered will be charged at the time of the deposit, or a minimum of 7 days in advance.  No refunds are available.
Parents will be responsible for any damages incurred during the scheduled party.
Payment is due the day of the party.  Payment may be made by cash, check, or credit card.
Please have your entire group together and ready to check-in at your designated check-in time.

A $60 NON-REFUNDABLE/NON-TRANSFERABLE deposit is required to secure your Birthday Party reservation. The deposit will be applied to your balance the day of your party.
