Registration
Information
- Complete Registration forms that can be picked up and return it to:

of Tempe Community Services Department, Social Services Office (2nd
floor of Library), 3500 S. Rural Rd, Tempe, AZ 85282
Office hours: Monday - Friday
8 am - 5pm

Select one
attendance schedule for a child to attend. Staffing does not permit the
flexibility of dealing with constantly changing schedules. (Exception
to this policy need to be authorized be the Kid Zone business office).
- Submit a completed registration form for each child you want to enroll
in the program along with a copy of the child's immunization record.
Please Note: Incoming kindergartners must be five years old by
August 31, 2011 for both the Kyrene School District and the Tempe School District.
If your child will turn five years old between the dates of September
1st, 2011 - December 31st, 2011, they may be able to test into
Kindergarten. For more information please contact the School
Districts.
- A non-refundable, registration fee per child is due at the
time of registration along with the first payment.

Parents are responsible to keep enrollment records
current including home phone number, full addresses, business phone, emergency
phone number, and child's scheduled attendance in Kid Zone. |

Enrollment
is available on a first-come, first served basis. The limit for student enrollment is primarily determined
by two factors. Programs are offered based upon availability of space at
each school site. Kid Zone programs service only the number of students
the space allows under DHS capacity limit guidelines.
