Sam was first met by Project H3 Vets at the Annual Maricopa County StandDown in March of 2013, he was referred by members of Tempe Outreach who brought him to the event. Sam was identified as chronically homeless, medically vulnerable, and was selected to be housed through the HUD-VASH program. Prior to StandDown Sam had never enrolled in services through the Phoenix VA. Stephen is an honorably discharged Veteran of the United States Army, serving from 1970-1972. Following StandDown Sam received temporary bridge housing assistance through the Arizona Coalition to End Homelessness at US Vets while he was assisted through the HUD-VASH housing process. Sam displayed an onset of memory loss and would return to the park he had previously been living at in Tempe quite often. CBI Peer Support Navigators worked with numerous members of the homeless provider community in Tempe to relocate Sam each time he wandered back to the park. With the collaboration of the Phoenix VA, Tempe Outreach, and CBI Sam moved into his own apartment in mid-March. He continued to return the park in Tempe, often forgetting the location of his apartment and being unable to return home as a result. Tempe outreach workers continued to assist him in getting home. Sam’s VA SW and CBI Navigators began to work together on getting his medical issues addressed as well as helping him transition into the apartment lifestyle. Sam suffered from a stroke a few months after being housed. Thankfully he had the support from many at this time and is currently recovering in an assisted living facility. He continues to receive assistance from the VA connecting him with long-term care as well as supportive services from his Navigation team at CBI.

*There are approximately 100 chronically homeless veterans remaining on the streets in Maricopa County reduced from 222 when the project started. Project H3 VETS will end chronic homelessness among veterans by June 2014.
