To request copies of public records from the City Clerk's Office, please fill out the Public Records Request Form below. Requests made for a commercial purpose require the Pricing Computation Form, as well. Completed forms may be mailed or hand-delivered to: 31 E. 5th Street - 2nd Floor, Tempe, Arizona, 85281. The forms may also be faxed to: (480) 858-2012, or e-mailed to: clerk@tempe.gov. The cost for hard copies of documents for non-commercial use is 25 cents per single page.
