Patient Advocate Services

In November 2013, Tempe Fire, Medical and Rescue Department and Tempe St. Luke’s Hospital launched the Patient Advocate Services Program. The goal of the pilot program is to provide patient-centered care and medical management services to certain chronically ill residents while reducing costs.

The Patient Advocate Services program is designed to help frequent emergency services clients while reducing costs and increasing coordination among healthcare providers. The program is unique to the Valley in identifying and assisting residents who could benefit from additional medical education, coaching and resources. The Patient Advocate Services team consists of a Tempe Fire, Medical, Rescue Department paramedics and a registered nurse from Tempe St. Luke’s Hospital. They have also partnered with local Tempe physicians to link patients lacking primary care to doctors. The program currently has 33 Tempe residents enrolled.

In December 2013, patient advocates visited 20 homes, made 26 patient follow-up visits and 10 physician contacts. The program also includes hospitalized patient visits, patient care records management, mental health and substance abuse referrals and patient medication research.
