Location Requirements - (Sec. 16A-133)
1.
can't be within 1,000 feet of another A.O.B.
2.
can't be within 1,000 feet of a Church
3.
can't be within 1,000 feet of a Child Care Facility, Preschool, Nursery,
any Elementary or Secondary School, Library, Public Park, Public Community
Building, Recreational Facility where minors are permitted, Liquor license
premise with #6 (Bar) or #7 (Beer & Wine Bar), Residential District, or the
property line of a lot devoted to a residential use.

Application packet must contain the following:
1.
application on the business
2.
individual applications of all owners, manager's, controlling persons
3.
copy of owners' and manager's driver's license
4.
$24 fee per person for fingerprint charge
5.
three small photos per person (can be taken by Licensing Staff)

Management agreement - (Sec. 16A-117(10))
·
Privilege tax permit
·
Proof of Use Permit
·
An accurate floor plan depicting the interior and layout of the
premises
1.
must show manager's station
2.
must show place at which the AOB license will be posted
3.
must show overhead lighting fixtures
4.
must show the portions of the premises in which patrons will not
be permitted
5.
must state the total floor space occupied by the business

certification that the AOB does not violate the location
requirements of Section 16A-133 of the code
·
All owners and managers are fingerprinted
·
